Speech Language Pathologist (SLP) – Home Health

Schedule / Job Type

Contract | Days | 4x8-Hour Shifts (8:00 AM – 4:30 PM) | No Weekends | No On-Call Required

Key Responsibilities

• Provide skilled speech-language pathology services to patients in a home health setting across the west Portland area and surrounding communities
• Conduct patient evaluations and deliver individualized therapy plans on a 1:1 basis, visiting up to 6 patients per day
• Complete clinical documentation using EPIC Remote Client and Rover mobile application, including OASIS assessments
• Meet standard productivity expectations of 30 productivity points per day
• Coordinate patient care and communicate effectively with the broader care team to support positive patient outcomes

Required Qualifications

• Active Oregon Speech Language Pathologist (SLP) license
• Minimum 1 year of skilled home health experience
• OASIS documentation experience required
• Basic Cardiac Life Support (BCLS) certification
• COVID-19 primary vaccination series plus one booster required
• Reliable personal transportation required; must be able to travel up to approximately 60 miles per day

Preferred Experience / Skills

• Experience with EPIC Remote Client for home health charting
• Familiarity with Rover mobile documentation application
• Comfort working independently in a home health environment with a geographically distributed caseload
