About the Position

Avanta Health is seeking a dedicated and patient-focused Primary Care Physician to join our medical practice in Fountain Valley, California. The physician will provide comprehensive primary care to adult patients, with an emphasis on preventive care, chronic disease management, continuity of care, and positive patient outcomes.

Key Responsibilities

• Provide comprehensive primary care, including examinations, diagnosis, treatment, and follow-up.
• Manage acute and chronic conditions, including diabetes, hypertension, COPD, cardiovascular disease, and other common conditions.
• Provide preventive care, annual wellness visits, health screenings, and patient education.
• Order, review, and follow up on laboratory tests, imaging, and other diagnostic studies.
• Prescribe medications and monitor treatment effectiveness.
• Maintain accurate and timely electronic health records and clinical documentation.
• Coordinate care with specialists, nurses, medical assistants, and other healthcare professionals.
• Participate in quality improvement, care management, and population health initiatives.
• Promote continuity of care and a positive patient experience.
• Perform other related duties and responsibilities as assigned based on the needs of the practice.

Required Qualifications

• MD or DO degree from an accredited institution.
• Completion of an accredited residency in Family Medicine, General Practice, or Internal Medicine.
• Board Certified or Board Eligible in Family Medicine or Internal Medicine.
• Current, unrestricted California physician license.
• Current DEA registration.
• Current BLS and ACLS certifications.
• New graduates completing residency are highly encouraged to apply.

 

Preferred Experience and Skills

Candidates who possess the following qualifications are encouraged to apply:

• Vietnamese-speaking ability is highly preferred. 
• Experience providing care to older adults and geriatric patients is a PLUS.
• Background in value-based care, managed care, community health, or similar healthcare environments.
• Strong clinical assessment and decision-making skills.
• Excellent communication and interpersonal skills.
• Ability to collaborate effectively with physicians, nurses, medical assistants, and other healthcare professionals.
• Commitment to preventive care, chronic disease management, and patient education.
• Demonstrated commitment to providing compassionate and culturally responsive patient care.

Compensation & Benefits

Salary: $200,000 – $400,000 per year

Compensation will be based on experience, qualifications, clinical background, schedule, and other job-related factors. Additional compensation, including potential sign-on or retention incentives, will be negotiated during onboarding.
